If you are not able to apply on-line and need to apply, e-mail registrar@ucsc.edu to request that an Announcement of Candidacy to Graduate* form be sent to you. Please use the following e-mail format:

On the Subject Line:

"Announcement of Candidacy Request"


In the body of the e-mail:

• Your name
• Your current address
• Your seven-digit student ID number


IMPORTANT

Students not enrolled at UCSC the current or one quarter prior to the quarter in which they apply will be assessed a $40 Records Reactivation Fee. UCSC Extension and Concurrent Enrollment programs are not considered UCSC enrollments.
