How to Obtain Access to Eval Web Faculty no longer need to request separate access for evaluations on the web. All teaching faculty may access evaluations for their courses on MyUCSC. Staff may obtain access to evaluations on the web by filling out the
appropriate forms. You can download
the account forms or get paper copies from your Service Center or
the ITS Support Center, 54 Kerr Hall, 459-4357.
The following forms are necessary: If you already have an ITS (name@ucsc.edu) e-mail account: If you do not have an ITS (name@ucsc.edu) e-mail account: File the completed forms with the Account Controller for your division, and keep a copy for your records. The Account Controller will forward the completed forms to the ITS Support Center, Kerr Hall, Room 54. You will be notified by ITS when your ITS account is ready (if you did not already have an ITS account). Due to the high volume expected, it may take up to 10 working days from the date you file the forms. You will then be notified by the Office of the Registrar when your Eval Web access is available. This notification will be sent via e-mail to your UCSC ITS e-mail account (name@ucsc.edu). The message will include the URL to access Eval Web (it is not published here for security reasons). If you need assistance, contact the ITS Support Center at 459-4357. If you need assistance setting up a web browser, contact the computing coordinator for your division. A list of computing coordinators is available on the web. Reminder: You are responsible for maintaining the confidentiality of student evaluations.
